Seiji Sawamura, also known as "Mad Dog", was an outcast. He stayed to himself and was misunderstood by everyone around him. Yes, he got into a lot of fights, but it was always to protect those who couldn't fight back themselves. He was one of those when he was younger, until his sister taught him how to fight back that is. Most everyone just assumed that Seiji was bad news and only cared about himself, so he didn't have many friends. He always wished for a girlfriend, but no girls wanted anything to do with him (and his approach needed a lot of work).
Midori Kasugano was a timid girl who had a major crush on Seiji. She watched him from afar for years, too scared to say anything to him. Midori wished more than anything that she could gather the strength inside of her to tell him her true feelings, but it never worked. She would always clam up again and watch as Seiji walked by, oblivious to the fact that she even existed. One day, Seiji woke up to find Midori had become his right hand and he was very frightened. Neither knew how or why it happened, but Midori was so happy to finally be able to talk to Seiji. She tried to be as helpful as she could to Seiji so she wouldn't be seen as a burden by him. She told him she loved him on a regular basis and annoyed him with her bubbly and quirky personality (as well as her handmade clothes professing her eternal love for him).
As Midori came out of her shell, Seiji began to change as well. He didn't fight as much because he didn't want to harm Midori or have anyone find out about her (though that didn't work very well). He lost a lot of his gruff persona and people began to see that he wasn't that bad of a guy. A lot of his classmates began to take notice of him as more than the class bully and he was finally able to make some real friends. One of Seiji's classmates, Takako, developed a crush on him after he saved her from some bullies, and tried her hardest to make him notice her. Though Seiji had wanted a girlfriend for so long, he was quite oblivious to Takako's, Midori's, and Lucy's (an American girl who decided that she was Seiji's girlfriend when he got her purse back from some bullies) advances. When Midori asked Seiji to tell her that he loved her, he wasn't able to. He had said the words to many other girls before, but he didn't really know them or the meaning of the words. By spending so much time with Midori, Seiji learned that those three words were very powerful and he couldn't just throw them around anymore. He learned to care not only for Midori, but about the feelings of his friends as well.
While Midori was having a great time as Seiji's hand and living out her greatest dreams (such as spending time in the park, just her and Seiji), her real body was in a coma. Her mother and friends were so worried about her and tried everything in their power to wake her up, and she did wake up once or twice, but she quickly returned to her dream world. Midori's childhood friend Kota sought out Seiji to see if he would be willing to help, but Midori didn't want to leave Seiji. Seiji also didn't want Midori to leave because she wouldn't remember anything about their time together when she went back to her real body. They were both happy and loved being together, but Midori knew that it couldn't go on forever. She had always dreamed about being the person she was while Seiji's hand, but it wasn't really her. Midori was a shy girl who couldn't muster up the strength to tell Seiji that she loved him in the past, and she hoped that when she went back to her body, her experiences with Seiji would give her that strength she so desperately wanted.

In the fifth volume of the manga, Midori is feeling a bit useless to Seiji. Being his right hand, she had to help him out a lot in the beginning, but he had since become really good at using his left hand for everything. She finally told Seiji what was bothering her, and he said this:
Seiji: "Jeez, you really have no clue, do you... listen, I'm only gonna say this once so listen carefully, okay!!"
Midori: "Hm?"
Seiji: "Don't worry about stupid stuff like that!! All you have to do is smile and just be there!!"

This quote is from the first episode of the anime, "Right Hand Girlfriend." Seiji isn't too thrilled with their situation, but Midori loves it. She has wished to be with him for so long and her dreams have finally come true!
Seiji: "Ughhh, this sucks, this really sucks."
Midori: "I don't know, I'm pretty happy right now."
Seiji: "Huh?"
Midori: "I mean, I get to be with you - the one I love so much!"

This quote is from the episode "First Date." After an eventful day of fending off Takako's advances on Seiji, Midori asked him to go on a walk. She led him to a park that held a special meaning for her and the two shared a special moment in front of the fountain.
Seiji: "I thought there was somewhere special you wanted to go. This is just the neighborhood park."
Midori: "Yeah, but it's my favorite place in the entire city!"
Seiji: "Huh?"
Midori: "The first time I came here was when I wanted to tell you how I felt, but back then, the most I could do was just gaze at you from afar. I thought wouldn't it be great if you and I could spend some time here together? And I'm so happy now because it's finally come true!"

This quote is from the episode "Takky Days." Midori is writing in her diary about her hopes for her future with Seiji.
Midori: "My number one is of course Seiji. No matter what happens, that won't change. That's why I hope someday, I'll be his number one."

In the end of "Hearts' Distance", Midori was watching Seiji sleep and he woke under her gaze. He was surprised to see her up, and she had something she wanted to say to him.
Seiji: "Hey, what are you doing up at this hour? Go on, get some sleep."
Midori: "Seiji?"
Seiji: "Hmm?"
Midori: "I just wanted to tell you that I love you Seiji."
Seiji: "I know that. You tell me that all the time. I'm going to sleep."
Midori: "Okay, goodnight then!"

In the last volume of the manga, Ayase finally told Seiji how she felt about him, but he turned her down and chose Midori instead. As they were finishing their walk home, Seiji decided to make his own confession to Midori.
Seiji: "You know, Midori...I...I know it's not like me to say this, but I used to think that what you need to liven up boring old school wasn't to play sports or join a lame club, but it was to find a girlfriend. That's why I'd fall for girls easily and quickly tried to make my move on them, but I kept getting rejected. To be honest with you, I couldn't give a crap after awhile, and it got to the point where I didn't care who it was, I just wanted a girlfriend."
Midori (thinking): "Seiji..."
Seiji: "But...I'm not like that anymore. I don't just want anybody...you're the only one I want as my girlfriend! Don't worry about the people around us!! I'll always protect you!! Always and forever I'll protect you!!"

This quote is also from the last volume of the manga. Midori finally had her dream date with Seiji and was prepared to return to her real body. When Seiji found out what she was going to do, he protested and tried to make her stay with him.
Seiji: "No, Midori! Please don't go! I love you Midori! I love you! You can't leave me! Stay with me like this forever! Please, I beg you, Midori!"
Midori (crying): "I...too...love you very much..."
Midori disappears from Seiji's hand

In the episode "Two People's Days," Seiji and Midori are separated. Midori doesn't remember anything about her time as Seiji's hand, but Kota tells her to go and tell Seiji how she feels. They finally find each other at the park they had gone to many times together and confess their feelings to each other.
Midori: "Uhhh, you might not know this, but... but I... I've watched you for a long time, and I... and I... I... I love you."
Seiji (smiling): "I do know. You know Midori, you've told me that a thousand times."
Midori: "There's no way! When? When was that?"
Seiji: "You see, well uhh... it's hard to explain, but I've watched you too. To be honest, I've watched you for a very long time, but it's not like I had much choice in the matter."
Midori: "Are you serious? You were watching me too? Does that mean?"
Seiji: "Yeah."
Midori: "Oh Seiji."
